When your care team feels that you have gained maximal mobility and are able to return home, the social worker and discharge planner will facilitate discharge. A multi-disciplinary Care Conference is held with family members to ensure that everyone is in agreement with the course of action recommended. The discharge planner will schedule any needed services, such as home health care and outpatient therapy, if necessary.
